How can I create a private form that is ONLY visible to agents?

By default, forms are visible on your End-User Portal(s) as they are a way for your end-users to submit tickets to your help desk team.

At times it may be necessary to create hidden forms that would not be accessible to your end-users but only to your agents when they are manually creating a ticket in the agent portal.  Hidden forms are NOT accessible on your end-user portal.
